* No charge is applied if the patient elects not to proceed with the suggested treatment. If the patient elects to take the suggested treatment, then the standard visit rate will be applied.
Forms:
The two most important forms you will be required to complete when visiting one of our clinics for the first time are the Patient Registration Form and the Patient Health History Form. To save time, it would be helpful if you print out these forms and complete them prior to your first visit. Please click on each form below to print it out.

Please bring your insurance card on your first visit if you intend to use your insurance.
If you wish to know if our services are covered on your insurance policy, please call our office ahead of your scheduled visit and we will check for you. The information you must provide to the office staff for checking your coverage is whether or not you belong to a PPO or HMO plan, your Member ID Number, First and Last Name and Date of Birth.
CORPORATE/COMPANY PACKAGE DEALS AVAILABLE !
If your company Insurance Plan does not cover Acupuncture or other Alternative Healthcare methods, contact your Human Resources Department and ask them to contact us about the possibility of contracting that coverage separately with our office. We do offer discount rates for such packages.
